Routine Immunizations
The CBE supports the AHS school-based immunization program.
Illness at School
Students who are ill and cannot remain in class should ask the teacher’s permission to report to the office. Parents will be contacted and the student sent home. All injuries and accidents involving students are to be reported to the office. The school cannot administer or be responsible for the administration of any medication to students.
Allergies/Medical Concerns
Please advise school administration of any significant medical problems. In particular, a Student Health Emergency Response Protocol Form must be filled out for all students with life threatening allergies, or those who use an Epi-Pen.
Medication
Student Health Emergency Response Protocol Form
This form is to be used when there is a student health issue regarding attention. This includes when there is a request to administer medication.
This form is to be:
- completed if a physical or medical condition may affect the student's attendance at school
- completed if medication is to be taken at school,
- reviewed and updated annually or sooner if there is a change in the student's health concern or school registration.
